Concrete Pad for a Carport

I need a new concrete pad in my existing two car carport. One side is ok (although it has some big cracks - I'd like these repaired as well)...the other side was never done and has old cracked paving stones. I would like a new concrete pad on this side. Please only apply if you are an experienced concrete drive contractor and can supply references.
